2. Attach a copy of the lead inspection report
3. Attach a copy of the Deleader Contractor License issued by the Division of Occupational Safety of the contractor you wish to hire
4. Attach copies of all estimates you received, sign the estimate from the contractor you wish to hire.
6. Attach a copy of all mortgage documents secured by the property to be deleaded
7. Attach a copy of the deed for the property to be deleaded
8. Attach copies of pay stubs for the last 8 weeks
9. Attach copies of your federal tax returns for the last 2 years
10. Attach copies of paid receipts for the most recent property tax payment, water payment, and sewer payment.
11. Attach a letter stating your intent to continue to occupy the property
12. Attach a copy of homeowners insurance binder
